What to Expect
When you first decide that the Chiltern Centre might be the right fit for you we will welcome you to come and have a look around and meet the team and some of our people we support. We will then ask you to complete a referral form so we can assess if we would be able to accommodate your unique needs.
Once we have read through this and learnt a little bit about you we will invite you for a home assessment – this involves us getting to know you a little bit better and ensuring that we all share the same values, aims and objectives. If you decide that the Chiltern Centre is right for you we can then start the process of transition so that you feel comfortable and at home at the Centre.
We will invite you for a minimum of 3 settling in sessions, ranging from a couple of hours to overnight. If you feel you need more time or a slower start to settle in then this can be accommodated as well. We always work with you as an individual and respect your needs and wishes. Once you feel settled and comfortable, we then get you booked in for your chosen days.
There is no ‘one size fits all’ when it comes to supporting people at the Chiltern Centre as everyone’s needs and likes are so vastly different; if you like to lie in until midday that’s fine by us! If you’re an early bird we’ll be ready to welcome the sun with you! All we need you to bring is your clothes, any medication you might need (we only administer prescribed medication so make sure they’ve come from the GP), if you’re PEG fed – your feeds and any toiletries and we will take care of the rest!
We enjoy an array of different outings and activities including arts and crafts, sensory stories, bowling, swimming, zoos, and farms etc and if you don’t fancy joining in then that’s fine too!

We welcome visits so you can experience what we do for yourself and talk to you more about how we can support you, your family or the young person you work with. With a range of services on offer we hope that we will be able to provide you with the support that you, your family member, or the young adult you are working with needs to live life to the full.
We take a holistic approach to supporting families and will tailor our service to each individual. We pace our introductions to suit each person, building up trust, friendship and confidence so that we can assist families in the best possible way for them. We look forward to hearing from you and hope to be able to support you over the coming months and years.
